Commercial insurance application

The ACORD 125 is the core application in a US commercial insurance submission. It identifies the applicant, the agency, and the insurer, and it says which line-of-business sections come with it, such as the ACORD 126 for general liability or the ACORD 140 for property.

What the ACORD 125 asks

  1. 01

    Agency, insurer, and policy

    The producer, the insurer and its NAIC code, the transaction type (quote, bind, issue, or change), the proposed effective and expiration dates, and the billing and payment plan.

  2. 02

    Lines of business

    The coverages requested and the sections attached for each. Underwriters check that every line ticked here has its section in the submission.

  3. 03

    Applicant

    The legal name, mailing address, entity type, FEIN, SIC or NAICS code, website, and the contacts for accounting, inspections, and claims.

  4. 04

    Premises

    Each location with its address, whether the applicant owns or rents it, employee counts, annual revenue, and floor areas. Location numbers here are the ones every other section refers to.

  5. 05

    Nature of business

    The type of business, the date it started, and a description of its operations. This is where an underwriter first decides whether the risk fits their appetite.

  6. 06

    General information

    Yes or no questions about subsidiaries, safety programs, hazardous materials, cover that was declined or cancelled, bankruptcies, judgments or liens, and foreign operations. Every yes needs an explanation.

  7. 07

    Prior carriers and loss history

    Previous insurers with their policy numbers and premiums, and every claim or loss for the years the form asks about, or a clear statement that there were none. Underwriters compare this part with the loss runs.

  8. 08

    Signatures

    The applicant and the producer sign and date the application. Most insurers will not bind cover on an unsigned application.

Gaps underwriters chase most

  • The applicant signature or its date is missing.
  • A line of business is ticked, but its section is not attached.
  • A general information question is answered yes with no explanation.
  • The loss history is left blank where it should say none.
  • Revenue or employee counts differ from the financial statements or another section.
  • The FEIN or the SIC or NAICS code is missing.
