What the ACORD 126 asks

Coverage and limits
Whether cover is on an occurrence or claims-made basis, and the limits: each occurrence, general aggregate, products and completed operations aggregate, personal and advertising injury, damage to rented premises, and medical expense. Deductibles sit here too.

Schedule of hazards
For each location, the classification and class code, the premium basis (such as gross sales, payroll, or area), and the exposure amount. The rating starts from this table.

Claims-made details
For claims-made cover, the retroactive date and whether prior acts are covered.

Products and completed operations
Products the applicant makes or sells, installation or repair work, and work given to subcontractors, with the questions underwriters ask about each.

Additional insureds and questions
Parties to be added as additional insureds, and yes or no questions about the operations. As on the ACORD 125, every yes needs an explanation.
Gaps underwriters chase most
- The exposure amount is missing for a class code.
- Gross sales in the schedule differ from the revenue on the ACORD 125.
- Claims-made cover is requested without a retroactive date.
- Subcontracted work is declared without details of the subcontractors.
